Azumaya Health membership gives you full access to all our services. Prices are listed per month and based on your age.

Direct Primary Care memberships can be used with or without insurance. We provide you with the care that you deserve.

Unfortunately, DPC does not work with all insurance plans. Some policies require patients to see an assigned physician assigned by the insurance company. These plans include HMOs and Medicaid. Talk with your insurance provider before signing up for a membership.​​​​​​

This type of appointment is for non-members. It can be used as an introductory visit for a transition to a future DPC membership. One-time visit applies to any situation and we can add additional charges if a procedure is required. Please send us a message below to schedule an appointment.

Having a big family can be loads of fun, but requires a tremendous amount of love and support. We support you and apply this discount if the family enrolls 4 or more members.

Our customers deserve the highest level of support, and we work tirelessly to maintain those standards. We understand that college students have a very fixed budget. Proof of schedule required for discount.
